Dr. Smith Retirement
After 36 years of medical practice with Associates in Family Medicine, Dr. Jerome Smith retired from AFM July 31 , 2011. Dr. Smith hosted a gathering open to all his patients and AFM thanks all of his loyal patients who attended to wish him well. Associates in Family Medicine was incredibly fortunate to have had Dr. Smith’s service for so many years. He served as the “gold standard” for family physicians in Fort Collins. Dr.
Lemay Office in Westbridge Medical Suites

We have moved and improved! Associates in Family Medicine’s oldest office at 1221 East Elizabeth has moved to 1107 S. Lemay, Suite 200, Fort Collins, CO 80524 as of April 21, 2009. The new office is located on the second floor of the new Westbridge Medical Suites building that is attached to Poudre Valley Hospital by a walkway. This beautiful new office space provides a convenient highly visible location on Lemay Avenue.
Foxtrail Family Medicine Clinic Serves Loveland Near MCR

- To better provide people in the booming I-25 and Highway 34 area with primary medical care, Associates in Family Medicine has teamed up with Poudre Valley Health System to open Foxtrail Family Medicine—just one block south of Medical Center of the Rockies.
Dr. Wideman Joins Horsetooth Staff
Dr. JD Wideman, DO, has been part of AFM for the past 2 years rotating between the various AFM offices. Effective January 1, 2011, she joined the staff at the 3519 Richmond Drive office. Dr. Wideman is accepting new patients. In addition to seeing the full spectrum of family medicine patients, excluding OB patients, Dr. Wideman also performs OMT.
New Physicians Join Timberline Staff
This summer AFM added 2 new physicians to our Timberline Office on Bighorn Road. Dr. Anne Siple, MD started at the Timberline Office on August 22nd. Dr. Siple moved from the Foxtrail Family Medicine Clinic where she has practiced for the past 2 years. Dr. Siple graduated from Northwestern University in Chicago and completed her Family Practice training at the Mayo Clinic in Rochester Minnesota.
